Hello,
I've created a new survey within service now with a simple numeric scale of 1-3. I've created a happy or sad face type email and would like the user to be able to click on the faces to give a response that corresponds to the 1,2 or 3 rating but I don't know how best to do this?
Has anyone done this before?

Hi,
If you want the response within the email, then you can create a UI page with URL parameter of their values.
So once anyone will hit the URL of let's say happy smiley face, it should call UI page & show the user that their response is received.
On the script side, you can call your survey instance record, create an entry in survey response with the value and that's how you can track their inputs.

Hi Gaurav,
We have similar requirement to this. When we click on smiley a new email gets popup for users response. However, is there any possibility where user just clicks on the smiley and feedback gets submitted.

Hi,
Even i have a similar requirement.When the user clicks on the smiley the survey should get submitted.
Is there a possibility of achieving this
