Portal question

Hello!
Happy new year!
I have question related to the portal. I have one form and need to add 2 links to it on the portal. each link will have different description. See attached image, the "Assurance" and the "Consultations".
The "Consultations" should take the user to the "Assurance" form, I need to add description to the "Consultations" one which is different from the "Assurance"
can I do that? and how?
The "Assurance" currently has "This form is used for......."

The screen shot you shared is look like 2 different Catalog items.
I have one form and need to add 2 links to it on the portal
Form mean> Catalog Item or something else?

So mate, you want to show these 2 terms as Link in one catalog item.
You can create the variable with URL TYPE
