It was pointed out to me today by a user that the "Add to cart" button is missing from all CIs in the portal
First I checked the catalog items to make sure "Hide" wasn't checked
Shopping Cart is still checked "visible"
Anyone have any ideas?

Hi,
As I'm sure you know, this doesn't just randomly happen.
First step would be to speak to your peers to see if anyone made any adjustments to this functionality. This would hopefully....be easier, than waiting for a response on the forums, and digging through documentation, etc. to try and figure this out.
Then, if you have no luck there...you can check on the service poral catalog item widget itself to see if it's disabled there (if the add to cart shows in the back-end UI, but not portal?). From your post it's unclear if you've looked into this beyond what you've showed.
Anyways, go to catalog item as if you're ordering it, ctrl+right-click on the catalog item form:
Choose "Instance Options". See if the "Show Add Cart Button" is unchecked?

That worked. When making a change like this and just checking a button, does that need to be done in an update set or can I just check it on dev and prod without making an update set?

Hello,
It's captured automatically in an update set (if you don't have one directly assigned, it's in the default set) and would consist of the "instance" of the SC Catalog Item widget, example:
