"User Form" in Playbooks - all fields are disabled

I am creating a simple playbook that just has 1 activity -- we want the user to update the Priority and Urgency on a record. In the playbook, I have added and configured the "User Form" activity. When I test it, the form displays correctly, and includes all the fields I selected, however all fields are disabled, and there is no Update/Save button.
We checked in a different environment where the User Form activity was working as expected, and it was configured exactly the same.
Any thoughts?
The playbook is in a scoped app.
The record is not read-only. (I do have access to update the record)

@Justin Lee2 Were you able to fix this, I'm facing the same issue in my Instance SN version Washington DC. It was working well in UTAH.
