Report on Risk Assessment values on Change Request

I have a requirement, questions which are part of Risk assessment in Change Request must be available in Columns for reporting purpose.

Not able to find the fields on Change Request table, questions will be shown in UI page when Risk Assessment related link is clicked.

The report on Risk assessment is not starlight forward.

You can create report on asmt_assessment_instance_question
