Setting incident as parent using UI Page

Hi All,

 

I have a requirement as below:

 

1. Need to create a UI action on incident form.

2. After user clicks on UI action ,a UI page should come with a reference to incident table where user can select an incident.

3. Once the incident is selected in UI page and user clicks on Ok, that incident should be set as parent of current opened incident where user clicked the UI action from.

 

Anyone has worked on this requirement, please suggest the solution.
