I'm afraid this is not possible in Survey Designer.

Is there any way i can do it outside the designer?

my scenario is like this …:

I want to send a HR task for user and in the task show him couple of his HR PROFILE details and also to collect new input from him about other things.

so i found out is the only solution for this scenario is “collect employee input” or “take a survey”

the two types open survey anyway.

I succeeded to show the user and also to collect from the user data, but as i showed in the photo its one under the other. And i have a lot of variables.

so if you have any solutions I will be very happy to hear.

thanks alot

Well, as a workaround, you could perhaps  consider a third task type: 'Submit Catalog Item'.

You could crate a catalog item for this that would not be part of any catalog or category, so it would not show on the portal etc. However you could map it to the HR task.

In the Catalog item, you can create a 2 column layout using containers, and even have some read only fields with values of the users HR profile etc., and other fields for capturing input. You can associate a flow to the catalog item to do something with the captured variables and perhaps even close the item immediately, etc.

Hi,

I tried what you said and everything worked beside that I couldn’t fill data from HR profile into the service catalog.

do you gave any idea how can I populate fields in catalog item dynamic  ?

only when I submit the catalog item in the task section of the portal I get the number of the requested item that is connected to the task
