Unable to See Right click Context Menu, Unclickable Magnifier glass, List lock/unlock is not working

Few of the table forms are behaving weirdly on my PDI. Whenever I am clicking on  Magnifier icon, nothing happens, no popup appears. Even List lock/unlock is not working.

Surprisingly Right Click context menus is not available for few forms. 

What may be the possible reason for such weird behavior? What needs to be checked here?

i don't think so. i am not 100 % sure . But  i have faced these issue before due to some customization on the form and it generally happen with some client side code which was blocking the right click on the form.

checked any UI Script, ui macro or ui page has been created recently adn used on your form ?

Not sure what was the actual issue, I just deactivated few recently created client scripts and it worked!!! I activated the client scripts again one by one to identify which one is causing the issue and I ended up with activating all of them. After activation of all client script the issue is no more.

Thanks, At last I found this issue is caused by one UI policy which is created on last month - after disabling that UI policy iam able to access Right-click context menu on form.

Dont know how but issue was fixed by doing instance Upgrade.
