When Category is Application show Configuration Item field on Propose a new Standard Change Template

Hello!

I want to display Configuration Item field when Category is Application on Propose a new Standard Change Template.

I tried UI Policy but it is not working on template. Is there anyway I can achieve this.

Please find screenshots below. All the help is appreciated.
