It is already present if you are referring to ESC Portal, you need to make it active=true. But yes, its mandatory. Until unless you don SYNC it the topics wont appear. You can try and see it when implementing the taxonomy.

I've added it it APN Table,
Now also I was able to see 2 level hirerechy only in the menu bar..
Topic -> Child Topic
But I was not able to see "Topic -> Child Topic -> Child Topic.."
is it expected?

I am assuming its ESC portal (OOB) . If you have added any topics and connected items make sure they are active true in menu items related list. Also try to sign out and sign in to see. The topics are visible only if they have content . And sync it aswell.
Also, if you are using custom portal , APN record doesn't appear and you may choose to skip it aswell. But i am not sure if skipping is best practice or not. For the rest please go through the documentation.
