What is your technical solution when creating a new look and feel for the Employee Center?

I've seen many examples of modern-looking Employee Centers and wondered how is it those are managed with the way Employee Center and Employee Center Pro is updated through the Store.
There is the out-of-the-box look and feel - that has widgets in certain places on pages, but those modern-looking employee center have different widgets and an overall remarkably different look and feel and layout. I understand that the css can be modified to achieve look and feel updates, but that doesn't help with the restructuring of where widgets are laid out or any new pages that may be created in a new layout.
How is that done? Are the OOB artifacts being cloned and the cloned versions being modified?
Is the entire Employee Center portal copied into its own version and that version being modified?
Then how are enhancements that we see in the updated store versions retrofitted into the modified versions?
