Survey instance missing Submit button

Hi

We have a survey instance created from FSM questionnaire. The assigned to Technician is missing the 'Submit' button from it despite all the mandatory questions are answered, however he sees the Submit button in the Portal, but not when the survey instance is launched from 'Questionnaire' UI action both on Mobile agent app and the Desktop. 

 

Does anyone one know what could be the possible cause here?
