Attestation - Attach evidence - attachment is lost causing an Issue to be generated

When a user takes an attestation:
- "Is this control implemented": Yes
- Attach Evidence - click paperclip
- Choose file: [file]
- The attachment now seems to be attached
- Add text to Explain field
- Submit
The Attestation is now Complete 100%, the Control is Compliant, but an Issue is generated and automatically closed: There was an Attestation failure. The attachment is not actually attached. This causes several emails to go out to the Control owner.
This issue is automatically closed
Log file shows: getEventTarget() called with invalid record reference: sys_attachment.b0c9e1be1b401d10addc86a5604bcb38 for event: attachment.uploaded, could have been deleted
Build tag: glide-sandiego-12-22-2021__patch1-hotfix1-03-25-2022
I cannot reproduce this issue in other San Diego instances (with higher patch levels).
Does anyone know if this a known issue in Quebec and San Diego patch 1?

I am not seeing the issue in either of my test instances. Here are the builds for each:
