Does anyone know if there is a way to have multiple contributors (respondents) to a single assessment instance?

While creating a risk assessment type and associating the entity, we'd like to have several contributors to ONE assessment instance to provide input collectively as a whole among a team .  Is this possible?

Yes You can!!

In terms of Risk is concerned , just Navigate to Risk> All Risks>Open the Risk as per your need.

In the Related Lists> Click on Assessment tab>in the "Assessment respondents" field , click on the "Lock icon"> Add your required users who should take up this assessment,  see below example:

This answer does not give you a single assessment instance:  I populates 5 totally separate assessment instances (see below):  I want one assessment instance that allows several contributors.

That's the expected Behavior!! It won't give you a single assessment instance, it should send  5 totally separate assessment instances .

Unfortunately, one assessment instance that allows several contributors is not possible as of now OOTB.
