How to hide Transferred to on hr portal

Hi Team,

Whenever I transfer a HR case I could see "HRC0012345 Cancelled - Transferred to HRC0012346" on the HR portal. 

"Transferred to HRC0012346" should not appear on the HR portal when a case is transferred. Could you please help/guide me.
