PDF Document for HR Service not Generating

I created a pdf document and want it to generate on a particular type of case. I have set everything up according to product documentation but it just continues to load and never generates. Does anyone know what the cause might be? Or does it look like I have something set up wrong?

Is automatically initiate document tasks case option selected on the HR service? Can you provide more details?
