Setting the fields on HR case from Employee form HR task

I have an employee form that a manager fills out and I want the fields they complete on the form to be copied over to case record. I set up the Mappings but it is not updating the case. I set up the filter conditions on the case as HR service = Requested Service and Subject person manager is dynamic me. This is not working. Is there a way to identify employee form is related to this hr case?
