Incident category & sub category or CSMB services, service offerings, application services?

Today , as on today 🙂

Which one should we use on incident form (considering new implementation of ITSM for a customer) -

category and subcategory 

or

Servies, offerings, application service, CI etc on the incident form?

I am a BPC and these discussion are part of my work.

Cat/ sub cat are different that Service / CI and offering.

Both are  equally important while incident creation. Cat/ sub cat tell you type of incident like H/w / S/W/ Application where Service/ CI tell which component is getting impacted or effected. 

CI/ Service play a vital role from that you can see the whole dependency view and effects.
