New field does not show on form until saved

I went to the table and created a new field 'Callback Number'.  I added it to the form view for Service Operations Workspace.  When I create a new incident it doesn't show until I save the record.  Any ideas?

There are 2 view for Service Operations Workspace.  Service Operations Workspace and Service Operations Workspace New Record.  Field needed to be set in each.
