Duplicated serial numbers being created

Discovery recently started creating duplicated serial numbers. There is no pattern on this that I could find so far. It does to different CI types and only to some CI, not all.
The first screenshot is for a windows server. I ran a discovery on another windows server in the same subnet but it didn't create duplicates. I couldn't copy the screenshot here, perhaps there is a limit of screenshots here.
Does anyone know or had experience on this before?

I had this similar problem, I tried clearing up the cmdb_serial_number table and re-ran discovery and it worked. You may try to perform the same in the non-prod instances first.

We are having similar problems as well for some Linux Redhat VMs (Model Name = Red Hat RHEV Hypervisor). Some just works fine. For those that have issues, we can see the pattern running twice of everything including trying to Create CI twice in the pattern log.
These discovery patterns logs also run into multiple pages.
We found this Recent Known Error affecting Vancouver/Washington which may explain the issue we are experiencing.
We suspect because there are duplicated payloads, the Pre-sensors maybe creating duplicate serial numbers and thus causing IRE issues.
