Incident not created for few alerts

I have got into a scenario where for some alerts the incident is not created.
The alert management rule filter matches properly and I can also see alert execution record in alert.
However there is no task/incident created out of it.
Kindly help me to find the root cause here.

Did you manage to solve this issue?
