The service now wiki talks about the Affected CI's related list but I can't find it when I try to personalize the forms.

Anyone know how to add the field that allows multiple CIs for a change, incident or problem ticket?

From your screenshot, you are looking at a new Incident record. Related Lists are not shown on new records. They will appear once you saved the record.
However, if it is truly required, you can use an Embedded List instead - Embedded Lists - ServiceNow Wiki

That worked thanks. I was hoping to open a problem, change or incident ticket that effected more then one CI and not have to save it first and then add the CIs but this is good enough.
