Attachment is missing in Email.

I have created a record procedure and added field called attachment and it is mandatory. while submitted the form I am adding attachment and submitted the form but in email notification the attachment is missing.
I selected check box as true as well i:e included attachment.
Notification condition is used when record is inserted  only and whom to send " customer note watch list " members. 
Kindly please help me it's very urgent.

After submitted the form under request details tab attachment is added in the portal while submitted i want same attachment should add in email notification
