Here's the workflow of the request associated to the catalog item:
Here's the RITM ticket created. No Catalog Task.

Hi,
On my side, in this reply chain, I'm working with you to resolve this and have asked those questions above to get us to this point.
This is the OOB workflow I'm talking about for Request:
Which I've attached here for you to download and import back in if you've changed it and can't find it on any other instance.
Please mark reply as Helpful/Correct, if applicable. Thanks!

Hi,
Click below & check for properties. It will give you the start conditions. Make sure it is met.
Also, go to UI Actions & then look for Show Workflow. Open it & add the Form Link option. Once done come to the RITM & you will find Show Workflow link below Worknotes. Click on that & see if the workflow as associated or not.

Sorry, but I don't see the Conditions tab in the Workflow Properties. When I am trying to load it it appears then disappears.
I tried all views but the Conditions tab is not showing up.
Also, nothing has been changed on the OOB UI action for show workflow under sc_req_item.
And Form Link is enabled:

Hi,
as you can see from your RITM record printscreen, the RITM stage is "Waiting for Approval", so you need to check first the sc_request workflow if you need an approval or not and configure it accordingly.
If I have answered your question, please mark my response as correct and/or helpful.
Thank you very much
Cheers
