change risk assessment

Hi,
In change request form I want the risk assessment link and questions to be enabled. I have enabled the risk assessment link for new record and in risk assessment condition I had added the filter condition as "state is new" but still I am not able to fill the risk assessment questions. Is there any other options to configure risk assessment questions for new change record?

There are Risk Conditions and Risk Assessments. The Risk Assessments out of the box use specific Categories in order to trigger. The Risk Conditions run whenever the risk calculations run.
So on Risk Assessments, did you verify your Change had the same conditions? Can you see the Related Link: Fill Out Risk Assessment?

Ill update here rather than starting a new thread.
I am having the same issue.
Yes, conditions are met.
No, there is not a "Fill Out Risk Assessment" related link.
There is a Risk Assessment related link, but this just pops up, gives me a window with the title of my assessment, but then says "You are not authorized to take this assessment?"
Any ideas would be appreciated.

The UI Action name may have changed over upgrades, but looking at a new jakarta instance it reads as Fill Out Risk Assessment:
Which checks Risk Assessments:
and based on conditions in the assessments, determines which one to show:
Risk Assessments should not run on New records, only after they have been submitted.
Have you customized Risk Assessment, or is it out of the box?

Hi Darren, did you ever get this resolved? I am also getting: "You are not authorized to take this assessment?"
on the Change - Risk Assessment link. I have admin and it'll_admin.
Anyone knows?
