Custom Approvals Best Practices (Flow Designer)

I had a question regarding Best Practices for changing Approvals. 

OOB: You ask for Approval on Flow Designer, it fires off an event, approval.inserted on the Approval's table, which sends out a notification - Approval Request. 

 

First question, if we wanted to change the theme/banner of the email we could change the template or the notification but the concern is that future upgrades would be skipped on template/notification. What's the best way to do it?

 

Second question, say that I wanted to create a custom approval for a Director for a Service Catalog item. I would use the Ask for Approval action on Flow Designer and create a new notification on the Approvals table. When the approval is requested however, it would fire off the "Approval Request" OOB notification and my custom notification. What's the best practice for this scenario? Only solution I could think of was to put in the conditions of the "Approval Request" that the cat_item is NOT my Director SC Item so the OOB notification wouldn't fire off but that leads to a bunch of problems such as the First question above and that you'd have to do this for all Custom Approvals.
