fields values are not saving on incident form

Hi All,
I am facing below issues with incident record.
I created one incident record manually, while creating the incident i selected caller field, assigned to, assignment group, and all other fileds information. when i click on submit/save button its showing record not found and some time am able to see that particular incident record but filed values are missing.
can anyone explain me what might be the reason to get this.

looks like incident creation process is abnormally terminated.
Check in logs if you see any error while creation of incident.
Also turn on the debugger to check the execution of business rules and go to specific one that might terminate the execution.
Please share the screenshot of error if you find any ; will be happy to help
