How to add catalog item name in sc_request form

Hi All,

I want to add the catalog item name field in the sc_request form. I have tried but it is not working. Can anyone give me a solution?

RITM workflow is not an option here as it gets triggered when the request is approved.
And looking at the requirement I believe it's mostly required for request approval.

You might not have approval on request.
I cheked again on my PDI instance and there is no workflow attached before the request is approved.
