How to remove ootb priority and impact and urgency
and also below thing in red

Hi,
This are task table fields. OOB fields, why you want to remove this fields. if you dont want to use this field just remove them from form.
Also check which display field is true on your incident table.

If you want to get it removed from form then you need to Right click form header & select Configure>>Form layout. If you want to break the Priority dependency which is on Impact & Urgency then you need to check for System Policy >> Rules >> Data look up defintions >> Priority look up & get it worked accordingly.
For the red marked option i.e. 3-Low it seems you have set the display value of the Incident table to either Impact or Urgency. Get it changed to Number.
