Not sure I understood your question. Do you need to report on Problem task due dates? You can do it directly like this:

What do you mean by due date extension? You can build a report and add it on the dashboard, or also configure the email notification to be sent to the assignee and assignment group about the approaching due date (similar to how we have it in KM with the ‘Valid To’ date).”

Need count of number of times Due Date has been changed.

Ok, please follow this steps

1)Create a Metric

2)Select table and field

3) This will not retrofit old records however it tracks when due date changes

ex:

 4)Create a report on "metric_instance" table where definition is due date updates as below and group by ID, it will show you the list with count
