Hi,
Just login as a Administrator and sort on Z to A on Created column.
From your screenshot you just need to Right click on the second record I can see which is the latest OOB one and then select "Revert to this version"
This will help you to revert it back to an OOB version and should resolve your issue.
Hope this helps. Please mark the answer as correct/helpful based on impact.

hello,
There is not problem with the way you are doing it.
Do you have the correct access to do the above action? Because when I tried on my PDI the method works?
Also have you just removed the fields from the form or deleted it?
Thanks.

1. check if you are in the correct scope, you might be trying in some other scope.
2. Elevate your role to security admin and try.
