Impact, Urgency and Priority Layout

I am wondering if any Healthcare Organizations would be willing to share the setup they are using for Impact, Urgency and Priority. We are looking at changing what we currently have setup and would like to hear some ideas other organizations are using today. Currently we have our Impact setup based on whether it affects patient care or not. We would like to include wording for staff who don't work in patient care areas like Accounting, Marketing, IS and etc. I would love to hear the wording others use.
Our Urgency is currently defined along time frames:
- High = 30 minutes or less response time
- Medium = 30 minutes to 3 hours response time
- Low = 3 plus hours response time
Below is what each scenario calculates to:
If Impact = low and Urgency = low then; Priority = 5 - Non Critical
If Impact = Low and Urgency = Medium then; Priority = 4 - Low
If Impact = Low and Urgency = High then; Priority = 3 - Moderate
If Impact = Medium and Urgency = Low then; Priority = 4 - Low
If Impact = Medium and Urgency = Medium then; Priority = 3 - Moderate
If Impact = Medium and Urgency = High then; Priority = 2 - High
If Impact = High and Urgency = Low then; Priority = 3 - Moderate
If Impact = High and Urgency = Medium then; Priority = 2 - High
If Impact = High and Urgency = High then; Priority = 1 - Critical

We have an App that uses something we created called a Hospital Services Map analagous to the Business Services Map but ties to how a Healthcare organization looks at their work. Would like to discuss with you offline. Contact me for a discussion.

So we use business service to set the urgency of our incidents, and then impact is manually set. I attached a word doc with our matrix in it so you can see. We also have two "over ride" check boxes. One is patient safety, one is work stoppage. Both will move the priority up one level. Since '1 - high" urgency implies patient safety, you can not check the patient safety box if high urgency is selected.
