KB Attached to the incident but not copied to KB task table.

There is an OOB functionality, when we attach any knowledge article to the incident, and it will automatically create a record in 'm2m_kb_task' table. But sometimes it is not getting created in (m2m_kb_task) table. Seems like an intermittent issue.
Does anyone aware of this or can anyone help me on this?
