Not sure y it is coming but may be u can check if there is any BR written on survey table. That is generally AddInfoMessage

I found one Before Update and Delete BR you can see below, but I am not sure it is related to that or not.

After getting the error shown by you did u cross check in the backend table whether the record got created or not by your inputs. If not please check the article provided by @Amit Verma that might be causing the issue.

May be the Scheduled Job "Cancel Expired Assessments" is the culprit here.
