"Next" button is not working for particular item in a order guide in Service Portal

Hi,

 

In a order guide 6 items is there. And this order guide is linked to the service portal.

 

All items in a Order Guide is working fine in instance. In Service Portal other 5 items working fine. I am able to submit that 5 items in Service Portal but, one particular item is not working("Next" button is not working) even though it is working in instance.

 

Can anyone tell what is the problem I am facing and how to resolve this issue?

Thanks for the reply Herrm... But it is not working for me! Still I can't submit the form... But in same order guide I am able to submit other items... only one item is creating problem
