RITM is created but SCTask not creating because of variables not being populated

I noticed that SCtasks are not being created for specific RITMS - see image below

I went into Flow Designer to see what the reason could be. I noticed that Output Data does not populate Application and Start Date and I think this may be the reason why the RITM is being created but not the associated SCTask
