root cause analysis methods in Problem record

why can't we have root cause analysis methods in Problem record?

Did any one implemented 5 Why's RCA methodology. it would be good to have separate module under problem section.

      Our Stakeholders are already looking for this implementation, not only with   5 Why, but also with Fishbone and Kepner Tregoe.


        Did you given a thought about how to implement ?


Service Now just recently announced the incorporation of the Kepner Tregoe method into the product; you may want to check into that...

Agreed, RCA should be part of problem.


Fields to fill in would be Cause code, Chronology of Events, Contributing Factors, Lessons learned, Recommendations

We created a section on our Problem records for the 5 Why 's RCA methodology. Our users like it, and it was very easy to implement.
