root cause analysis methods in Problem record

why can't we have root cause analysis methods in Problem record?

Did any one implemented 5 Why's RCA methodology. it would be good to have separate module under problem section.

Can you please give details regarding how you created RCA methodology?   I'm new to Service Now and I got the requirement to create RCA in fish bone method. Your inputs are valuable.

I realize that this is a little old but I have a Federal customer who is also wanting to do the same thing while leveraging the "5 W's". Is there any screenshots or details on what was done for this RCA Function? Thank you in advance for anything you can share.


We just created a field for each why and answer with a solution field, then added those fields to a new section on the problem form. There wasn't any scripts or any complexity to it - just a place to store the 5 whys on a problem.
