We took the Fishbone, the 5 Why's and a 'reduced' version of the Kepner Trego and had them created into Tabs. Based on the RCA tool you select at the top, the appropriate Tab will populate with fields found on any of the 3 tools. We started with forms/templates that Problem Coordinators were to use, then scan and attach to the Problem Records, but they weren't complying. So we made it mandatory and added a new field "Root Cause Tool" and provided the same 3 options. It has worked, although the 'forms' aren't pretty in the tab, but we can now report on which tool is being used.
