UI Policies not working in RITM and Catalog task level.

I have created like around 13 UI Policies for my catalog form and i have checked the box applies to RITM & catalog task.
And i have also disabled all the client scripts written. But still the catalog variables aren't reflecting in my ritm or task tickets.
For example:
When i fill in all these details..
I get only these in my ticket.

Hi there,
It should be able to work. Can you share how you set your Catalog UI Policy up? And I assume a Catalog UI Policy, not a UI Policy?
Do the 2 screenshots also represent the same record? Asking this because for example "Is it a Generic Account?" shows different values. One filled, one --None--
For example, how did you set this up:
