ui actions not visible

I created a created and a form and my ui actions are not being recorded on the form.I see ui actions are not visible when i try to search in configure..can someone help?

I have created a table, did form design, process flow,applied few UI policies and have a requirement to make a mandatory field to pop up in separate small pop-up having ok and cancel options and saying assigned to is mandatory

I have created a UI page using script.

I can want to connect UI Page created to the form using UI action so that requirement can be met

but when I am trying to configure and go to UI Action..it is all empty ,it shows no UI Actions

Can you help me with his ?
