Inconsistency with Now Assist bot responses

We have Now Assist for VA installed in our DEV instance. We are noticing inconsistency with Now Assist creating RITM's for the existing conversational catalog items we have in DEV. We get this response from Now Assist VA- "Sorry, there was a problem on my side trying to complete this request. Try asking again later."
Any help is appreciated!

are you able to identify what topic/utility is responsible for this? At what steps it is happening?

Probably conversational blueprints and categories. For example, asked Now Assist to open an Incident for keyboard issues and this is the response I received -
'Sorry, there was a problem on my side trying to complete this request. Try asking again later."
There are no agents available at the moment. Please try again later.
We have not yet made live agents enabled.
