my ui action buttons are not working

in my incident table my mandatory field caller is not working .i a unable to acess the reference tables and my created ui action buttons are not working

in incidents when i try to create a new form in that page i am unable to select caller refence field or i am unable to select any refernce fields available in that form .in other tables i am able to do that. in incidents i am also unable to do a right click on field

when i released my instance it is working normally
