As a change manager, we are assuming that some changes happening in couple of CI's without change manager knowledge and proper change request. So we just want to know How we can check the unauthorized change happened in any CI's without Change request. what would be the steps to be followed to get the unauthorized change report.

I completely agreed. Actually what I am looking here is How we can monitor and avoid if any changes performed by any individual (Not by the system) without any change requests in order to streamline the process.
