how to create a multiple fields x axis and y axis as a range 0 to 10 report for a user performance of a record in servicenow

Hi @Manisha Goli 

In Multi level pivot table you can add multiple field in row and column i.e x and Y axis like below

There are multiple type of report are available in OOTB like Pivot / Multi Pivot / Bar / charts. Please check those which one fit in your bucket.
