I recently took over ownership of an ServiceNow Instance, there are very minimum document about the instance, are there any document or guideline on how to run some assessment about the instance and document it?
1) what are the integration to/from the instance?
2) Service Account?
3) Modules Install on the platform?

Greetings!!
The first thing you can refer to is Now Create and there you will get some documents which help you to understand the instance.
1) what is the integration to/from the instance?
To check the integrations to and from a ServiceNow instance, navigate to System Web Services > Outbound > REST Message within your ServiceNow instance; this will display a list of configured outbound integrations, allowing you to see which external systems are currently connected to send data out from your ServiceNow instance.
2) Service Account?
To check service accounts in ServiceNow, navigate to the "System Users" module, where you can filter the list by checking the "Web Service Access Only" checkbox, which indicates a user is designated as a service account; you can also identify service accounts by looking for users created specifically for integrations and with limited permissions to access the user interface directly.
3) Modules Install on the platform?
Contacting your ServiceNow sales team is the best and most reliable way to get this information. However, you can also check out subscription management in your production instance for some information on your own.

Thanks, are there any specific documents which can be refered to do this activity? those 3 questions are something I thought I need to look-up and capture, anything else?

I don’t have anything readily available, but I believe if you search for these keywords in the ServiceNow documentation, you should find the relevant information.
