New and create problem task UI button is not working in problem SOW

New button in the problem task related record in problem form and create problem task UI button in problem form in SOW is not working. Can anyone please help on this?

May be your page is not loaded properly, it should be look like this:

Thank you for your response. However, this doesn’t appear to be a loading issue. When the button is clicked, nothing gets populated even after waiting for a long time. Just for your information, we are not using the out-of-the-box (OOB) functionality for the Problem module.
