Ask For Approval "All users approve" - Flow Designer Action

Hello All,

We're facing a weird error on our Flow,

There's 3 people on the group "Y_CHG.0001" , the flow is supposedly configured to mark the "Ask for Approval" action as completed and then go to the next step.

The strange behavior happening is that when the first person on this group approves it, the flow understands it as 'completed' and goes to the next action.
Have you guys been here before? Is the Action properly configured? Any Ideas on how to solve it?

I replicated the same on my personal instance and it seems to be working alright on my instance. Can I request you to check the following, please?

Are the remaining users active who are in the group?

Do they have the right roles in approving a change_task record?

Is the flow activated with 'All users approve' change on the flow?

If they all seem in line, please share a screenshot of execution like this so we can dig a bit more -

by the way, I checked all the items you have recomended and the answer for all the questions is YES...

Not a whole lot on the flow execution but just wanted to make sure the approval flow was set to approval all on the condition. 

Did you try with a different group because this is supposed to work as you have configured so no problem with that?

Also, try copying the flow and see if it's the same on the copied flow as well.
