Create Subflows & Actions: Build a Subflow - Washington - Flow not creating Problem Record

Hello All, 

I have completed creating the flow for the 'Create a Subflow' activity. But for what ever reason, the flow is not creating the Problem record. Looking over the Execution Details, the condition is 3=0, I have no idea how to fix it so it does not read the condition as such. Any suggestions?

Hi,

In 4th step if condition check the count of (3rd) look up records action in 3rd step. 

As per the execution it is checking for (1st) look up records action in 1st step
