Creating survey with radio button

Hi Experts,

I need to create survey with following points,an end user should receive an email with embedded survey with the below content :

  • Introduction : Your ticket has been resolved. Please help us improve by taking our short satisfaction survey .
  • The questions will remain the same as existing survey but the options will be different for the question "Please rate overall quality of the service provided" the radio buttons should show :
    Completely satisfied, Moderately satisfied and Not at all satisfied.
  • User should be able to click on the radio button to select his option and that survey should be then recorded with ticket number so that we can fetch the report to check the survey response status.

Hi,

 

1. You need to create a new survey.

Survey --> View Surveys and open New Survey

Fill the required fields and in the top right you have an option called Add Questions. You can add whatever questions you want to display in the survey.

Click on Add Questions. Then it will open another form. 

In Assessment, metrics click new

You have to select Data Type as Field Selection.

You have to publish the survey once you have completed your questions.

 

If you want to set any trigger condition you have to set it in the Trigger Conditions Section.

 

At last, the survey will come in below manner.

This embedded survey mail should go along with the resolved notification mail that user receives 

How can i achieve this ?

On which table this notification will trigger?

 

If you are looking in the Incident table we have Out of the box Notification is there.

 

System Notifications --> Notifications  

Search for Incident Survey.
